Contact UsOur team is always out and about in the community, supporting people to take their first step to live well. We attend community events, promoting our free support service to residents. We also have an NHS Health Check team who go out in the community to deliver targeted NHS health checks to help residents find out if they are at risk of cardiovascular disease and identify what lifestyle changes they can make to reduce their risk. So, take a look at where our team will be this month and come and chat with us!
